Forms for Animal Housing, Import & Export, and Movement on Campus

All forms may be sent to Melissa Forrestier in the Comparative Medicine Office. They may be sent via email, fax or delivered to BMSB 203.

Comparative Medicine - Housing Request Form

Use this form (one form per strain) when ordering animals to be housed in the Rodent Barrier Facility or the Conventional Facility. This is the only form needed when ordering animals from commercial sources. Incomplete information will result in a delay in approval of the order.

 

Animal Transfer Request Form

Use this form when acquiring animals from or sending animals to a non-commercial source, i.e. other universities. This form is to be used in conjunction with the Housing Form.

On-Campus Transfer Form

Use this form when moving animals to any other location on the OUHSC Campus; i.e. room to room, building to building. This form should also be used when changing account numbers and/or protocol numbers.
