Occupational Health Program

Risk Assess Form:  Animal Handlers Health Questionnaire

Purpose

An occupational health program is essential for personnel who work in laboratory animal facilities, or for those whose work requires them to handle, maintain or use animals for research or teaching. The University of Oklahoma Health Sciences Center receives Public Health Service funding, is registered with the USDA, and as an institution accredited by the Association for the Assessment and Accreditation of Laboratory Animal Care (AAALAC, International) is required to have an occupational health and safety program. The following procedures and guidelines are in place to promote the health and safety of all personnel involved in animal care and handling. This manual is available online at:

http://www.ouhsc.edu/iacuc/OccupationalHealthProgram.asp

The Occupational Health and Safety in the Care and Use of Research Animals manual is meant to provide a condensed reference describing the OUHSC laboratory animal occupational health program. Much of the information presented in this manual is included in the University of Oklahoma Health Sciences Center Laboratory Safety Manual at: http://w3.ouhsc.edu/ehso/Normanlabman/NormanLaboratorySafetyManual.htm
