This team training module is one of a series of self-instructional modules in geriatric medicine offered by The Roy M. and Phyllis Gough Huffington Center on Aging, Baylor College of Medicine, Houston, Texas. The initial creation of the Huffington hypertext modules was supported by the John A. Hartford Foundation of New York initiative entitled, "Geriatrics in Primary Care Residency Training."

This module was supported by a grant of the Geriatric Interdisciplinary Team Training Initiative from the John A. Hartford Foundation, Inc. The John A. Hartford Foundation, Inc. The Foundation created the Geriatric Interdisciplinary Team Training Initiative (GITT) and awarded grants to partnerships between academic institutions and clinical providers. Beginning in 1997 eight models were funded to implement and evaluate ways in which to develop the team skills of physicians, nurses, social workers, and other health care professionals. This team module is a product of the Houston GITT project supported by a grant from the Foundation to Baylor College of Medicine’s Huffington Center on Aging. The New York University Resource Center serves as the national coordinating center and GITT program office for the eight project sites.
