 University of Oklahoma Protection of Health Information Policies and Procedures Manual
The Policies contained in this manual are intended to provide guidance to University Personnel in regard to the protection and enhancement of the privacy rights of patients by (a) establishing rules related to the internal and external use and disclosure of protected health information; (b) affording patients access and information regarding the use and disclosure of their protected health information; and (c) implementing administrative procedures intended to assist patients and University Personnel to effectuate these Policies.

 OUHSC PHI Server Consolidation Policy
Servers containing ePHI must be consolidated into the campus enterprise data center. The data center will provide physical and environmental protections for the security and privacy of these confidential and mission-critical information assets.

 Notice of Privacy Practices
The University's Notice of Privacy Practices summarizes all the ways in which the University might use a patient's protected health information and informs patients of their rights with respect to the protected health information.
