IMPORTANT INFORMATION REGARDING SUBMISSION OF A NEW PROJECT APPLICATION
- List only individuals on the new project application (Principal Investigator, Co-Principal Investigator, sub-investigator, collaborator, or research coordinator) who qualify under the following definition of key personnel:
Individuals who provide study-related services or assistance, who have knowledge of the study protocol AND the identity of study participants or their protected health information.
- The education status of all individuals indicated on the new project application will be verified upon receipt by the IRB Office. Applications will be returned if the following required education has not been completed:
Required education for new* key personnel:
- Register for the next scheduled IRB In-House Education Program
- Completion of the CITI Basic Web-based Course
Required education for current** key personnel:
- Attendance of the IRB In-House Education Program
- Completion of the CITI Basic or Refresher Web-based Course
- Written Attestation, signed & dated, on file in the IRB Office
*New key personnel are those individuals submitting to the IRB for the first time.
**Current key personnel are those individuals who have previously submitted to the IRB.
