The Harold Hamm Oklahoma Diabetes Center offers many treatment and education opportunities for patients who are
concerned with preventing or controlling Diabetes.
The Harold Hamm Oklahoma Diabetes Center is an American Diabetes Association Nationally Recognized Diabetes
Education Program Provider. We provide group and individual consultations on diabetes prevention/management, diabetes weight loss, cardiovascular disease and diabetes, renal disease
and diabetes, insulin pump therapy and carbohydrate counting. We also provide a high risk foot treatment
program and continuous glucose monitoring. All of our classes are taught by Certified Diabetes Educators (CDE's).
Physician Referral Form
The Physician
referral form is required to obtain service reimbursement from insurance companies. Please print out one of these forms (depending on the services needed), have your physician's office fill out the information and fax it back to us at (405) 271-1012.

Insulin Pumps
Insulin Pumps are a new and innovative way to treat diabetes. The Harold Hamm Oklahoma Diabetes Center takes pride in our 2 insulin pump trainers. Continuous Glucose Monitoring System (CGMS)
CGMS is a service offered by the Harold Hamm Oklahoma Diabetes center that allows the physician to obtain a minute by minute reading of their blood sugars for 72 hours. This technology allows the patient and their healthcare provider determine the best course of action in treating their diabetes.
