Visiting the OU Cancer Institute

What should I bring for my first visit?

If you were mailed a packet by the clinic that will be seeing you, be sure to fill out required paperwork and follow the directions sent to you. In general, good things to bring with you are:

  • Original films
    • Mammograms, X-rays, MRI's, CT scans or PET scans
    • You may be given a CD instead of actual films.
  • A List of Your Medications and Doses
    • Be sure to also include any herbal supplements or over-the-counter medications
  • Medical History
    • Know information such as other illnesses, surgery dates, etc.
  • Contact information
    • Office and fax numbers for your referring physician and other providers you want to keep updated on your care
    • Emergency contact information
  • Insurance Information
    • Including Medicare/Medicaid information
  • Health Care Directive
    • An advance directive is just a statement of how we want to be cared for in situations where we can't make decisions on our own. This is something good for all of us to do, but is required for any surgeries at the OU Medical Center. To download a simple worksheet, click here.
