Answer and Discussion: The answer is (D). Usually Kaposi sarcoma shows spindle cells with minimal nuclear atypia. Scattered plasma cells, extravasated erythrocytes, and hemosiderin deposits are common findings. The lack of high grade atypia made Kaposi sarcoma a difficult particularly when the lesion is small.
