If you have lost your COVID-19 vaccination card or you would like a separate document to carry with you, you can print an imunization record from OSIIS. OSIIS is the Oklahoma State Immunization Information System, which is a centralized immunization registry that keeps electronic records of vaccines given by health care providers who use the registry.
