The OUHSC Police Department provides copies of police reports in compliance with the Oklahoma Open Records Act.  Releases are processed through the University of Oklahoma Open Records Office.  To make a request, fill out the form and return it by mail, email, fax or in-person to the OUHSC Police Department:
